Assessment
We begin with a personal assessment to determine your needs and preferences. During this time, we explain the different senior care options and resources available. If needed, we provide you with financial and legal resources.

Selection
We share the top three senior care living options and services that match your profile. We will schedule a tour of your top three selections. We will personally meet you and your family at each location to ensure you have an excellent experience and are then able to make a fully informed decision.

Placement
Once you have selected your new Home Sweet Home, we provide you with the guidance necessary for a smooth transition. We will partner and care for you as if you were a member of our family! We will facilitate communication between caregivers, communities and health care providers. Once you have been placed in your new home, we follow-up with you to ensure your expectations and needs are met.
